Days - 6-month assignment.
Must have experience in authorizations and referrals and case management more so than clinical.
* *SUMMARY: *Identifies, plans, and coordinates high-risk patient care (frail, chronic disease, catastrophic injury) in an ambulatory setting.
Reviews referrals and applies the UM standards for patient care.
Ensures continuity of care across the continuum.
Promotes and evaluates quality and cost-effective outcomes.
- Previous experience in healthcare, preferably in nursing or behavioral health - Knowledge of managed care principles and practices - Familiarity with medical terminology and HIPAA regulations - Strong understanding of acute care, primary care, and pediatric services - Ability to work effectively in a hospital or clinical setting - Excellent communication and interpersonal skills *QUALIFICATIONS: *To perform this job successfully, an individual must be able to perform each essential function satisfactorily.

*CERTIFICATES, LICENSES, AND REGISTRATIONS:* Current California LVN license.
*LANGUAGE SKILLS: *Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
Ability to write routine reports and correspondence.
Ability to speak effectively before groups of customers or employees of the organization.
*WORK ENVIRONMENT:* The noise level in the work environment is usually moderate.
